The DHCS Home Safety Checklist

1. Fall Prevention (The #1 Priority)

Falls are the leading cause of injury for seniors and recovering patients. We recommend these modifications:

  • Clear Pathways: Remove all “trip hazards” such as loose rugs, cluttered hallways, and electrical cords.
  • Anti-Slip Surfaces: Use non-slip mats in the bathroom, kitchen, and any tiled areas.
  • Enhanced Lighting: Install motion-sensor lights in hallways and ensure nightlights are used in bathrooms and bedrooms to prevent disorientation in the dark.

2. Bathroom Safety

The bathroom is often the most dangerous room due to slippery surfaces.

  • Grab Bars: Install high-load-bearing grab bars near the toilet and inside the shower.
  • Raised Toilet Seats: These reduce the strain on hips and knees for patients with limited mobility.
  • Shower Chairs: Allow patients to bathe safely while seated, reducing the risk of dizziness or slipping.

3. Bedroom & Mobility Support

A safe bedroom promotes better rest and safer transitions.

  • Bed Height Adjustment: Ensure the bed is at a height where the patient’s feet touch the floor comfortably when sitting on the edge.
  • Support Rails: Install bedside rails to assist with sitting up and getting out of bed.
  • Firm Support: Use an appropriate mattress (such as an air mattress for bedridden patients) to prevent pressure sores.

4. Medical & Emergency Readiness

  • Accessible Communication: Keep a phone or emergency alert button within reach of the patient at all times.
  • Clear Labeling: Ensure all medications are clearly labeled and stored in a cool, dry place away from children or confused patients.
  • Equipment Maintenance: Regularly check that wheelchairs, oxygen concentrators, and hospital beds are functioning correctly and have no frayed wires.
